When Counseling Supports Interventional Treatments
When counseling alone does not provide sufficient relief, a psychiatrist may review symptoms, past diagnoses, and prior treatment. This is especially important for people with treatment-resistant depression, PTSD, or anxiety disorders that interfere with daily life. Remember that counseling still plays an important role, but it can become part of a broader interventional psychiatry plan. A psychiatric evaluation helps determine when other treatment options deserve consideration.
When symptoms continue despite consistent counseling
Ongoing symptoms may warrant revisiting the treatment plan. A patient may continue to experience low mood, panic, intrusive thoughts, sleep problems, low motivation, or difficulty functioning even while attending counseling sessions consistently. In these situations, the psychiatrist can review how long symptoms have lasted, which treatments have already been tried, and whether another diagnosis or medical factor may be contributing. Counseling does not need to stop just because the plan expands; it can continue alongside other therapies when appropriate.
When depression does not respond to standard treatment
Treatment-resistant depression (TRD) generally refers to depression that does not improve enough after adequate trials of more than one antidepressant. The psychiatrist may discuss interventional options rather than relying only on repeated medication changes, such as:
- TMS. Uses magnetic pulses to stimulate specific brain areas involved in mood regulation.
- PrTMS. Uses a more personalized approach to repetitive magnetic stimulation.
- Spravato (esketamine). Medication in the form of a nasal spray that patients must take in the office over the course of a few weeks to months.
Mental health is personal to everyone. Therefore, when determining the most appropriate treatment plan or ways to help, especially for TRD, the psychiatrist considers the diagnosis, prior response, medical history, and treatment goals before recommending any services.
When counseling becomes part of interventional care
As aforementioned, patients benefit from keeping counseling connected to interventional treatment, as its main aim is to help them process emotions, notice patterns, practice coping strategies, and discuss changes that emerge during care. Ketamine-assisted psychotherapy takes this integration a step further by pairing psychotherapy with ketamine administration under professional guidance. Ketamine itself is used off-label for psychiatric conditions, so the psychiatrist should explain the potential benefits, risks, alternatives, and monitoring involved during an appointment. Bear in mind that the exact role of counseling may differ between ketamine, TMS, PrTMS, and Spravato treatment plans.
When PTSD or anxiety symptoms remain difficult to manage
Post-traumatic stress disorder (PTSD) and anxiety disorders can also require a more specialized evaluation when symptoms remain severe despite standard care. Trauma-focused counseling often remains important for PTSD, while counseling can also support anxiety treatment. Interventional psychiatry services may be considered in selected cases, particularly when treatment-resistant depression or anxiety symptoms overlap, or other care has not provided sufficient relief. The psychiatrist will clarify which uses are FDA-approved, which are off-label, and how each option fits the individual diagnosis, keeping treatment centered on safety, realistic expectations, and the symptoms causing the greatest disruption.

Persistent symptoms do not mean the patient has gone through all treatment options. Our psychiatrist, who has expertise in treatment-resistant depression and anxiety disorders, can review previous medications, counseling, diagnoses, and current symptoms before discussing options such as TMS, PrTMS, Spravato, ketamine, or ketamine-assisted psychotherapy.
